Epiminder advances US epilepsy rollout with DETECT enrolment ahead of plan

Medical Devices and Digital Health By Victor Sage 4 min read

Epiminder has enrolled its 100th patient in the DETECT US epilepsy study ahead of schedule, while maintaining A$65.1 million in cash and no borrowings. The company says those reserves can fund clinical development and commercialisation work well into 2028.

  • 100th DETECT patient enrolled on 15 October, ahead of the November target
  • Study recruiting across 23 US centres, with up to 210 participants planned
  • Next-generation Minder device remains on track for design completion in 1H CY2027
  • A$65.1 million cash and no borrowings at 30 September 2026
  • FY27 cash outflow forecast of approximately A$45 million to A$48 million

DETECT enrolment reaches 100 patients ahead of target

Epiminder Limited (ASX:EPI) has reached a key clinical milestone earlier than planned, enrolling the 100th participant in its DETECT study on 15 October. The company had previously targeted that milestone for November, and now expects to reach 125 patients by December 2026 before completing enrolment of up to 210 participants by the end of the first half of calendar 2027.

The study is recruiting through 23 US epilepsy centres, including leading institutions such as Stanford, Yale and several Mayo Clinic sites. The acceleration builds on the 75-patient DETECT milestone, reached in September. Epiminder says DETECT is now past halfway, although the headline 100-patient figure represents less than half of the stated 210-participant upper limit; the filing does not explain that difference.

Clinical evidence remains central to US market access

DETECT is designed to test whether continuous EEG monitoring with the Minder implantable system is superior to standard care in identifying clinically actionable events among people with drug-resistant epilepsy. Its results are intended to support clinical practice, payer engagement and Epiminder’s broader US commercial strategy.

No DETECT efficacy or safety results were reported in the update. The immediate milestone is therefore recruitment rather than a clinical readout, with the study’s primary endpoint focused on the proportion of participants achieving an actionable clinical event. The company’s commercial case still depends on completing the study and converting the resulting evidence into physician and payer adoption.

Next-generation Minder device moves into testing

Epiminder has manufactured approximately 200 next-generation devices for biocompatibility and functional testing. The G1 software is feature complete for the planned 510(k) predicate, with stabilisation and testing now under way.

The next-generation device is intended to support easier implantation through a slimmer telemetry unit, while also offering advanced communications, greater data storage, longer battery life and lower manufacturing costs. Design completion remains targeted for the first half of calendar 2027, with FDA clearance targeted for the end of 2027. Those dates are company targets rather than completed regulatory outcomes.

A$65.1 million cash balance funds the next phase

Epiminder held A$65.1 million in cash and no borrowings at 30 September 2026. The balance is consistent with the quarterly cash update, which also said the company expected its funds to support DETECT and next-generation development into 2028.

The AGM presentation forecasts FY27 cash outflow of approximately A$45 million to A$48 million, reflecting peak investment in the clinical study and device program. Epiminder says those costs should reduce materially from FY28, although the financial information in the presentation is unaudited and the forecast excludes any FY27 research and development tax rebate.

Medicare pricing and a 2028 launch remain ahead

The company also pointed to a preliminary CY2027 Medicare reimbursement amount of US$31,600 for the relevant hospital procedure, up from US$27,700 in 2026. The final CY2027 rule was expected in November 2026, leaving the preliminary figure subject to change. Epiminder says an additional Transitional Pass-Through payment opportunity may also be available.

Management is targeting early commercial opportunities through pharmaceutical research, the US Veterans Health Administration, individual patient reimbursement and Australian authorised-prescriber arrangements. A broader US commercial launch is planned for the first half of calendar 2028. Before then, investors have a sequence of execution tests to track: 125 DETECT patients by December, full enrolment in 1H CY2027, next-generation device completion, FDA review and the final Medicare payment decision.

Bottom Line?

The enrolment pace is encouraging, but the investment case now turns on whether clinical evidence, reimbursement and device development advance together without stretching the cash plan.

Questions in the middle?

  • Can Epiminder sustain recruitment to reach 125 patients by December and complete the 210-participant study in 1H CY2027?
  • Will the final CY2027 Medicare rule preserve the preliminary US$31,600 reimbursement level?
  • Can next-generation device testing and FDA clearance stay aligned with the planned H1 CY2028 commercial launch?
