Alterity Clears Key FDA Hurdle as ATH434 Heads Toward Phase 3

Alterity Therapeutics says it has begun operational preparations for a Phase 3 ATH434 trial after FDA minutes confirmed the proposed design and registrational pathway in Multiple System Atrophy. The company ended September with A$41.3 million in cash, while a new US patent extends protection for ATH434 to 2045.

  • FDA alignment on a single pivotal Phase 3 trial plus confirmatory evidence
  • Approximately 200-patient study planned for ATH434 in Multiple System Atrophy
  • New US composition of matter patent extends protection to 2045
  • A$41.3 million cash balance after option proceeds and R&D tax refund
  • Phase 3 funding and partnering alternatives remain under evaluation

FDA Confirms Proposed ATH434 Phase 3 Pathway

Alterity Therapeutics (ASX:ATH, NASDAQ: ATHE) has moved ATH434 closer to a pivotal trial in Multiple System Atrophy after receiving the FDA’s final End-of-Phase 2 meeting minutes. The regulator agreed with the proposed study population, twice-daily treatment regimen, primary endpoint and key secondary endpoints, according to the company.

The FDA indicated that a single pivotal trial, supported by confirmatory evidence, could provide the data needed for a potential New Drug Application. Alterity expects its ATH434-201 Phase 2 data to provide that confirmatory evidence. That is a meaningful regulatory clarification, but it is not an approval or a guarantee that the eventual data will support registration. The development follows the earlier FDA Phase 3 pathway, which had described a single pivotal trial backed by prior Phase 2 evidence.

Approximately 200 Patients Planned for Pivotal Trial

The planned Phase 3 study is expected to enrol about 200 patients, randomised equally to ATH434 50 milligrams or matching placebo, administered twice daily for 12 months. Its primary endpoint will be the 11-item UMSARS Part I rating scale, a functional measure covering activities of daily living affected by MSA.

Operational work has begun, including protocol finalisation, vendor selection, clinical-site identification and qualification, and manufacture of the first registration batch of ATH434. Alterity also reported a company-presented analysis at the October MDS Congress in which the 50 milligram dose was associated with an approximately 52% slowing of functional decline versus placebo at Week 52. That analysis used statistical methods planned for Phase 3, but it remains evidence from an earlier Phase 2 study rather than a result from the planned pivotal trial.

Patent Extends ATH434 Protection to 2045

A US composition of matter patent granted in August covers a crystalline mesylate salt form of ATH434 used in the Phase 2 trials and planned for Phase 3, along with methods for treating neurological conditions. The patent expires in 2045 and is expected to be eligible for listing in the FDA’s Orange Book if ATH434 receives regulatory approval.

The new protection adds to Alterity’s existing intellectual property and orphan-drug designations in the US and European Union. The company says the patent also supports potential future development in Parkinson’s disease and other conditions involving iron dysregulation. The award extends the US ATH434 patent previously reported ahead of Phase 3, strengthening the asset’s stated long-term protection but not resolving the clinical and regulatory risks still ahead.

A$41.3 Million Cash Buffer Meets Uncertain Funding Needs

Alterity held A$41.308 million in cash and cash equivalents at 30 September, after receiving a A$3.983 million Australian R&D tax refund and raising approximately A$5.5 million net from ATHO option exercises and the underwritten shortfall. The quarter’s net operating cash outflow was A$1.339 million, while the Appendix 4C calculates 30.8 quarters of funding at that reported rate.

That runway calculation is mechanical rather than a forecast for a Phase 3 program, whose costs may differ materially from the quarter’s spending. Alterity continues to assess funding and partnering alternatives with external advisers and pharmaceutical companies. The capital raised through the underwritten ATHO options is earmarked for late-stage development and working capital, giving the company more time to advance preparations while leaving the larger financing question unresolved.

Bottom Line?

The regulatory route is clearer and the balance sheet is better funded, but the next value-defining steps are site activation, enrolment and a credible plan for financing the full Phase 3 program.

Questions in the middle?

  • When will the Phase 3 protocol be finalised and the first clinical sites activated?
  • Will Alterity secure a pharmaceutical partner or raise additional capital before Phase 3 spending accelerates?
  • Can the Phase 2 signal be reproduced across the planned 200-patient pivotal study?
