Guidance rises after an $111 billion quarter

McKesson Corporation (NYSE:MCK) has raised its fiscal 2027 adjusted earnings outlook after another strong quarter, lifting the range to US$45.60-US$46.40 per diluted share from US$44.20-US$45.00. The company reported second-quarter revenue of US$111.0 billion, up 8% year on year, while GAAP diluted earnings per share rose 17% to US$10.44 and adjusted diluted EPS increased 18% to US$11.68.

The upgrade is based on adjusted earnings, a non-GAAP measure, and McKesson does not provide forward-looking GAAP EPS guidance. The company said it cannot reasonably forecast items including LIFO inventory adjustments, litigation contingencies, transaction costs, restructuring and impairment charges. That distinction matters: the reported quarter included US$85 million of restructuring, impairment and related charges, alongside transaction-related costs linked partly to the planned separation of Medical-Surgical Solutions.

Specialty healthcare drives operating momentum

Growth was concentrated in the company’s core pharmaceutical and specialty-care operations. North American Pharmaceutical revenue rose 6% to US$91.4 billion, with adjusted segment operating profit increasing 22% to US$1.0 billion as specialty-product distribution to health systems and strategic accounts expanded.

Oncology & Multispecialty was the faster-growing operation, with revenue up 24% to US$15.0 billion and adjusted segment operating profit up 8% to US$427 million. Prescription Technology Solutions also produced an 18% increase in adjusted operating profit, although Medical-Surgical Solutions moved the other way: its adjusted operating profit fell 10% to US$225 million, with the company citing product mix and weaker contributions from the ambulatory care channel.

Capital returns and portfolio expansion compete for cash

McKesson generated US$3.1 billion in operating cash flow and US$2.9 billion in free cash flow during the quarter. Over the first six months of the fiscal year, it returned US$3.6 billion to shareholders, including US$3.4 billion of share repurchases and US$198 million of dividends. The diluted share count was down 6% from a year earlier, which helped support the per-share result.

At the same time, the company is putting capital into a broader healthcare-services platform. It agreed to invest about US$1.4 billion for an approximately 49% stake in Option Care Health, subject to shareholder and regulatory approvals, and completed its acquisition of Precision Medicine Group. McKesson also extended its pharmaceutical distribution partnership with CVS Health through June 2032 and expanded the PRISM Vision Group network through the addition of Spectrum Vision Partners.

Medical-Surgical separation remains a key execution test

The quarter’s headline numbers are favourable, but the portfolio reshaping is not cost-free. McKesson recorded US$114 million of transaction-related charges in the first six months associated with the planned Medical-Surgical Solutions separation, while adjusted operating profit in that segment declined. The Option Care Health investment also remains unfinished until its stated closing conditions are met.

That leaves two separate questions for the next phase: whether the specialty and services expansion can sustain the raised earnings range, and whether management can execute several transactions and the Medical-Surgical separation without allowing one-off costs or weaker segment margins to absorb more of the operating gains.

Bottom Line?

The raised adjusted EPS range signals confidence in the core business, but delivery now depends on specialty growth and disciplined execution across multiple portfolio changes.

Questions in the middle?

  • Can Oncology & Multispecialty maintain its growth rate as McKesson integrates Precision Medicine Group and expands specialty services?
  • Will the Option Care Health investment receive the required approvals and contribute on the anticipated timetable?
  • Can Medical-Surgical Solutions recover its margin performance while McKesson funds and executes the planned separation?
